Multi-Discipline Outpatient Program

SIRH’s Multi-Discipline Outpatient Therapy program helps people who need multiple therapies per visit. At the hospital location, a supervised area called Day Hospital is available. Some patients benefit from a shorter day that allows therapies to be scheduled closely, while other patients benefit from rest breaks between therapies to fully benefit from treatment.

When needed, Nursing supervision is available with training on medical issues such as diabetic teaching and wound care. The Rehab Team ensures that each patient's treatment plan is specific to his or her needs.

Multi-discipline outpatient’s have access to physician consultations as needed. Patient’s following a stroke are referred to the SIRH Stroke Clinic while other patients who may need medical follow-up have access to SIRH’s Medical Director who will work in conjunction with the therapy team and the patient’s referring physician to maximize the patient’s outcomes.

The assistance of a Case Manager is available to help those attempting to access community resources for transition to discharge. Case Managers provide information ranging from home care assistance to accessing community social services.

Multi-discipline outpatient therapy is designed to be flexible to meet the varied needs of our patients. Treatment may vary in frequency of days and the therapy needed. For example, one patient may tolerate physical, occupational and speech therapies being scheduled one after the other, while a patient who has suffered a stroke might require physical, occupational and speech therapies to be scheduled with a rest break. Additionally, a patient may need to arrive for therapy early if his family’s transportation is limited, and go home during the lunch hour. SIRH offers a supervised area known as Day Hospital where the patient may remain while waiting for his/her therapy and prior to going home for the day.
